The role of the renin-angiotensin system in the regulation of the peripheral vascular resistance was studied in 13 patients with chronic heart failure, administering the converting enzyme inhibitor SQ 14225 (Captopril,25 or 50mg, P. 0.).

Plasma renin activity (PRA) before administra-tion was 0.1-7.5ng/ml/h. The cases were divided on the basis of PRA before administration into two groups, high PRA group (PRA>2 ng/ml/h) and low PRA group (PRA < 2 ng/ml/h). In the high PRA group, after giving Captopril, PRA and angiotensin-I increased significantly (4.1 ± 0.7 to 38.6 ± 15. 2 ng/ml/h, 1866±319 to 6053±1647 pg/ml, respectively), but plasma aldosterone and norepinephrine decreased (15.6 ± 3.9 to 6.3±1.5 ng/dl, 0.33±0.10 to 0.25 ± 0.07 ng/ml, respectively). In the low PRA group, however, these hormonal responses to Captopril were not significant.
