グリチルリチンによる偽アルドステロン症に合併した心室粗細動の1例

A case of ventricular flutter and fibrillation due to glycyrrhizin induced psendoaldosteronism

　甘草（Licorice）は，本邦では漢方薬として広く使用されており，その有効成分であるグリチルリチンは，抗アレルギー剤，肝疾患治療薬として用いられている。

　この甘草あるいはグルチルリチン大量投与による副作用として，偽アルドステロン症の生じることが知られており，本邦では7例の報告がある^1〜7）。

A 67-year-old female who had history of essential hypertension since February 1966 was admitted to local hospital on 27th of Oct. 1977, because of suspected hepatitis. Her serum electrolytes were Na 147 mEq/l, K 3.5 mEq/l on admission. Glycyrrhizin 200 mg/day in DIV for9 days was done. Around 12: 00 on 4th of Nov., syncope for one minute developed, when she was singing a song in lavatory. She recovered her consciousness without any medication.

Second syncope which was recorded ventricular flutter and fibrillation on monitor EKG developed 7 hours after the initial episode. This was successfully treated with external cardioversion.
