Flail Pulmonic Valveの超音波所見

Cross sectional and M-mode echocardiographic features of Flail Pulmonic Valve

　心エコー図による細菌性心内膜炎の診断は，弁に附着する疣贅や弁組織の破壊像を記録することによってなされる。特に後者の場合はflail aortic valve^1），flail mitral valve^2）とよばれ，支持を失った弁が血流によって，大動脈，左室流出路，左房に翻転する状態を表現している。今回我々は，細菌性心内膜炎による肺動脈弁前尖の高度破壊像，すなわちflail pulmonic valveを超音被検査法にて診断し，剖検にて確認されたので報告する。

Although there have increased the number of reports which indicated an increased frequency of tricuspid valve involvement by bacterial endo-carditis, pulmonary valve involvement has been thought to be uncommon.

This report descreibed cross-sectional and M-mode echocardiographic findings of flail pulmonic valve caused by bacterial endocarditis. The M-mode echocardiogram demonstrated unusual, fluttering echo pattern in the outflow tract of the right ventricle in diastole which were shown to be continuous with fluttering pulmonary leaflet in diastole.
