人工透析における除水の肺機能に及ぼす影響についての検討

Pulmonary Function with Acute Loss of Excess Lung Water by Hemodialysis

　慢性腎不全で長期透析中の患者は，週2〜3回の透析時の除水によって体液調節が行なわれているわけであり，この体液過剰状態，および，透析時の急激な除水が肺に何らかの変化を起こすことが想像される。1973年，Zidulka^1）らは，透析後にclosing volumeの減少することを報告している。われわれはclosing volumeとならんで，肺の末梢気道の病変をdetectすると考えられているflow-volume curve^2,3）について測定し，また，透析後の下降脚の改善の程度と透析時の除水量との相関関係について検索した。

We studied lung function in nine patients with advanced renal failure who were on a chronic hemodialysis program. Before and after hemo-dialysis, we measured lung volume, 3-Hz oscillation respiratory resistance and flow-volume curve. All patients had almost normal value in conventional lung function studies. The V₂₅ and V₅₀ in flow-volume curve increased with hemodialysis. The increasing rate had correlation to the volume of water lost with the hemodialysis.
