外傷性subclavian steal syndrome

Traumatic subclavian steal syndrome

　脳および上肢の虚血性症状を呈するsubclavian steal syndromeの手術報告例は，欧米ではすでに多数みられ^1,2,3），本邦でも近年大動脈炎症候群に随伴した症例^4,5）のほか，動脈硬化によるものが次第に増加の傾向にある^6,7,8）。しかし，外傷に起因したsubclavian steal syndromeは非常に稀で，その報告例は欧米でも数例^9,10）にすぎず，本邦ではいまだにみられない。最近我々は，鈍的外力による本症候群の一例を経験し，外科的治療により治癒せしめたので報告し，併せて若干の文献的考察を試みる。

A case of subclavian steal syndrome of traumatic origin is reported. This 41-year-old man who had been caught in a lift and suffered a crushing type injury to the chest developed brachio-basilar insufficiency about three months following injury, produced by thrombogenic obstruction of the innominate artery secondary to dissection of the intima, and was treated by thromboendarterectomy of the injured portion of the innominate.
