成人にみられた右肺動脈上行大動脈起始兼左動脈管開存の1臨床例

Origin of the Right Pulmonary Artery from the Ascending Aorta Associated with Patent Left Ductus Arteriosus

　右肺動脈大動脈起始症は，他の心奇形を伴うことも，単独にみられる事もあるが，多くは幼小児期に死亡し，成人例はごく稀である。本報告例は，成人まで生存しえた上，α—blockade （phenoxybenzamine）の使用によって臨床症状および他覚的所見の改善をみた。その成因，病態生理，治療について若干の考察を加えて報告する。

A 25 year-old female patient with the right pulmonary artery originating from the ascending aorta and patent left ductus arteriosus is reported. Since Frantzel's autopsy report about origin of the right pulmonary artery from the ascending aorta in 1868, 80 cases have been reported so far, 12 of them in Japan.

When we consider that with only five exceptions these cases were in new borns or infants, our case can be said to be quite rare.
