緒言
慢性肺気腫のガス及び血液動態に関してはSe—agal1), Motley2),村尾3),4),5),横山6)等により既に多数報ぜられている。元来肺の機能は気相及び血液相との間におけるO2とCO2のガス交換が主である。然も血液相は動脈血によつて或程度代表され得るので気相及び動脈血の各検査成績を求めることによつて相当正確な肺機能障害の成績が求め得るわけである。著者はこの中CO2を主体とし,各種肺機能検査の一環としてListon—Becker社製Model 16CO2メーターを使用して得られた検査成績を中心として述べる。
Studies on alveolar air in 80 patients with chronic pulmonary emphysema were performed by use of Liston-Becker continuous CO2 analyzer. Arterial blood CO2 tension was similarly examined in 59 cases among the total. In the previous days alveolar functions were studied on the basis of the theory of alveolar equation, which reveals that the CO2 tensions of alveolar air equals to that of arterial blood in steady state. But in pulmonary emphysema we could make it clear by continuously analyzing CO2 gas concentration in the expired gas that the inhaled air, even most diffusible CO2 gas, was unevenly distributed in the emphysematous lung tissues. It is, however, necessary to recall that these relations between uneven CO2 distribution and the factors of functional disability of the lung has been also evaluated from the so-called "Ideal Alveolar Equation", and hence these probelms should be resolved in the future studies.
