 Diaphragmatic paralysis(DP)is a complication after cardiac surgery for congenital heart disease. We retrospectively analyzed incidence, surgical details, management and follow-up of our patients with DP after cardiac surgery performed between 2005 and 2011. The incidence of DP was 1.8%(27 patients). Median age at cardiac surgery was 5 months(range 0 month to 7 years). Diaphragms of 10 patients were fixed and those of 9 patients moved paradoxically. Diaphragmatic plication was performed in 21 patients at a median interval from the cardiac surgery of 16 days(range 6 days to 180 days). In 6 patients without plication, the diaphragm came to show a normal position during follow-up. Patients who needed plication were significantly younger and had experienced previous cardiac surgery(p<0.01). For patients with DP, diagnosis,management and follow-up should be carried out appropriately.
