BAL (bronchoalveolar lavage) is a bronchoscopic technique to retrieve epithelial lining fluid for examination of cellular and humoral components that are present in distal airspace. A sufficient amount of isotonic saline is instilled into a subsegmental bronchus and recovered. In patients with interstitial pneumonias, BAL fluid usually shows increased cell counts and certain patterns of differentials that can be consistent with specific types of interstitial pneumonias. In the supernatant of BAL fluid, levels of various mediators, such as inflammatory cytokines, chemokines, and growth factors, are elevated. New tools that use genomic and proteomic characterisation of BAL cells and soluble components may eventually prove useful in making an accurate diagnosis and in monitoring disease activity and therapeutic response.
