　症例は74歳，男性．1992年4月から慢性腎不全のため血液透析を開始され，1994年10月から腹膜透析導入となった．1995年9月胸部X線写真にて粒状・斑状陰影が認められ，肺胞蛋白症と診断された．ambroxol hydrochloride内服を試みたが，1996年12月より胸部X線写真の陰影が増悪し，1997年3月再入院した。慢性腎不全により，循環動態が不安定と考えられたため，経皮的心肺補助（percutaneous cardiopulmonary support：PCPS）併用による肺洗浄を施行し，症状の改善をみた．腹膜透析中に発症し，肺洗浄におけるPCPSの有用性が示唆された肺胞蛋白症の1例を経験したので報告する．

A 74-year-old man suffering from chronic renal fail-ure (CRF) was started on hemodialysis in April, 1992, and continuous ambulatory peritoneal dialysis (CAM) was started in October, 1994. Pulmonary alveolar proteinosis (PAP) was diagnosed in September, 1995. He was admitted in March, 1996 because the pulmonary infiltrates on his roentgenogram had become worse. His symptoms improved after whole lung lavage ,using percutaneous cardiopulmonary support (PCPS), was carried out. The combination of WLL and PCPS proved useful in treating PAP with CRF.
