非乳頭部十二指腸神経内分泌腫瘍（NET）の内視鏡診断からみた治療方針

The Endoscopic Diagnosis and Treatment of Non-ampullary Duodenal Neuroendocrine Tumor（NET）

要旨●十二指腸の神経内分泌腫瘍（NET）はまれな疾患だが，早期発見が増えたことで内視鏡治療を検討する症例は増加している．内視鏡治療はエビデンスが十分ではなく研究的治療として位置付けられており，腫瘍径10mm未満かつ深達度が粘膜下層（SM）までにとどまる病変が基本的な適応とされている．内視鏡治療は，穿孔や出血などの偶発症や深部断端陽性となるリスクが高く，近年では腹腔鏡・内視鏡合同手術（D-LECS）が重要な治療選択肢となっている．十二指腸NETはコンセンサスが得られていない事項も多く，施設間でのポリシーも異なるのが現状であり，今後のさらなるデータの集積が望まれる．

　Duodenal neuroendocrine tumors（NETs）are rare, but the number of cases considering endoscopic resection is increasing. Currently categorized as an investigational treatment due to insufficient evidence, it is generally indicated for tumors <10mm in size and confined to the submucosal layer. However, endoscopic resection poses a high risk of complications, such as perforation and bleeding. Consequently, laparoscopy endoscopy cooperative surgery for duodenal neoplasm（D-LECS） has become an important treatment option. Many aspects of duodenal NETs lack consensus, leading to varying policies among hospitals. Therefore, further data accumulation is expected in the future to address these uncertainties.
