 Gastric MALT(mucosa-associated lymphoid tissue)lymphoma has various presentations on endoscopic images, sometimes making it difficult to differentiate from gastric cancer. In particular, undifferentiated-type early-stage gastric cancer and adenocarcinoma of the fundic gland type show signs similar to those of gastric MALT lymphoma. To differentiate from undifferentiated-type early-stage gastric cancer, findings on the tumor border and the presence or absence of encroachment should be evaluated. In contrast, to differentiate from adenocarcinoma of fundic gland type, gastric MALT lymphoma exhibits differences in lesion size and distribution. Furthermore, MALT lymphoma often exhibits multiple lesions within the stomach, whereas gastric cancer is primarily solitary. In addition, a characteristic vascular structure(tree-like appearance)in magnified observation is also an important point for differentiation. In this article, we describe the endoscopic images and discuss key points in the pathological diagnosis of gastric MALT lymphoma, focusing on its differentiation from gastric cancer.
