 The 5-year survival rate of colorectal T1 cancers is 〜97% ; thus, many patients recover with proper treatment. Conversely, there are rare cases with a poor prognosis. In this study, we analyzed the long-term course of 826 patients with colorectal T1 cancers treated at our hospital between January 2005 and December 2016, and examined the clinicopathological characteristics of patients with simultaneous or heterogeneous distant metastasis. Multivariate analysis showed that venous invasion was a major risk factor for distant metastasis of T1 cancers. Moreover, T1 cancers with positive venous invasion had significantly lower disease-specific and recurrence-free survival rates than those with negative venous invasion. Thus, even in T1 cancers resected with R0, distant metastasis should be carefully considered in patients with positive venous invasion.
