 The current endoscopic diagnosis of colorectal cancers ranges from normal to image-enhanced and magnified observations due to the development of endoscopic equipment. Establishing an area of interest and creating a detailed diagnosis is necessary while understanding the overview of each case. Therefore, it is important to correctly diagnose the lesion and decide the treatment plan to avoid overtriage in the patient.

 Determining the presence of advanced VI or VN findings in the pit pattern to identify the possibility of endoscopic resection is important in cases with no obvious evidence of advanced submucosal invasive carcinoma on normal observations, if the patient is Japan Narrow-band imaging Expert Team type 2B or equivalent. Herein, we present an actual case of endoscopic diagnosis of normal adenoma and adenocarcinoma.
