臨床医の考える臨床画像と病理所見の対比—大腸

Clinicians' Perspectives on Comparison between Clinical Images and Pathological Findings in the Large Intestine

要旨●大腸における病変の内視鏡像を含む臨床画像と病理組織像を対比することは，臨床の術前画像診断が切除検体の病理診断に対して正しかったのか，誤っていたならばどこがどのように誤っていたのか，病理診断の結果のみならず，症例の振り返りとして極めて重要である．そのためには関心領域における適切な内視鏡像を撮像すること，適切な位置に割が入るよう切り出しを行うことが必要である．臨床医自らが切り出し業務に関わりつつ，病理医と十分に連携をとって関心領域を正確に切片に出すことで，既知の診断の確認，そして新しい診断学の発展の一助になりうる．

　The comparison of clinical images, particularly endoscopic images of large intestinal lesions, with pathological specimens is crucial not only to validate pathological diagnoses but also to review cases and identify discrepancies between the clinical and histological diagnoses of resected specimens. Hence, capturing precise endoscopic images and ensuring accurate sectioning of specimens are necessary to facilitate appropriate specimen placement for analysis. Clinicians actively participate in the sectioning process and, in collaboration with the pathologist, accurately sectioned the region of interest, thereby confirming diagnoses and helping develop new diagnostics.
