 Reports, particularly from Western countries, have often suggested an epidemiological shift in the incidence and distribution of cancers within the esophagus and stomach over the past decades, from distal sites in the stomach(adenocarcinoma)and proximal sites in the esophagus(squamous cell carcinoma)toward the esophagogastric junction(EGJ adenocarcinoma). On the other hand, a report on the long-term trends of EGJ tumors showed no evidence of any increase in EGJ adenocarcinoma in Japan. Changes in the prevalence of risk factors, such as obesity, Helicobacter pylori infection, gastroesophageal reflux, and Barrett esophagus, may contribute to explaining this shift, although no definite conclusion has been drawn. Further research is needed to elucidate the risk factors and mechanisms of carcinogenesis.
