 Gastric adenocarcinoma of gastric phenotype is classified into GAFG(gastric adenocarcinoma of fundic gland type), GAFGM(gastric adenocarcinoma of fundic gland mucosal type), gastric adenocarcinoma of foveolar type(whitish, flatly elevated type ; raspberry shape type), and gastric adenocarcinoma of gastric phenotype with H. pylori(Helicobacter pylori)infection. We observed that gastric adenocarcinoma of gastric phenotype exhibited some peculiar endoscopic and demarcation findings. For instance, GAFG and mixed type of differentiated and undifferentiated gastric adenocarcinoma with H. pylori infection, in which the cancer had not spread to the superficial mucosal layer, were not demarcated clearly either by WLI(white light image)or ME-NBI(magnifying endoscopy with narrow-band imaging). In contrast, GAFGM, gastric adenocarcinoma of foveolar type, and differentiated type gastric adenocarcinoma with H. pylori infection, in which the cancer had spread to the superficial mucosal layer, were clearly demarcated by both WLI and ME-NBI. We could classify the endoscopic features of gastric adenocarcinoma of gastric phenotype according to H. pylori infection status. It is necessary to evaluate and assess the endoscopic characteristics of each tumor, including the type of background mucosa, for precise endoscopic diagnosis and demarcation of gastric adenocarcinoma of gastric phenotype.
