要旨●胃がんリスク層別化検診（ABC検診）のD群からは高率にA型胃炎（AIG）が発見された（6/24）．しかし，2016年のABC検診の改訂基準ではH. pylori抗体価の陰性カットオフ値は3U/ml未満に変更されたため，今後はAIGの多くがC・D群に属することが想定される．いずれにしても，AIGはPGIおよびPGI/II比が著しく低値であるという特性を理解しておくことが，発見契機のうえでも重要となる．また，D群として発見されたAIG例を提示しながら，本邦でのAIG共同研究の結果から得られた内視鏡診断の留意点を紹介した．

　In the gastric cancer screening method, the so-called “ABC method”, using the combination assay of Helicobacter pylori IgG antibody and serum PG（pepsinogen）levels, 25% individuals were diagnosed with AIG（autoimmune gastritis）and classified as group D. However, after adopting the revised criteria for this screening method, the individuals with AIG were expected to be categorized in either group C or D. Very low PG levels and PG I/II ratio are key to diagnosing AIG. An AIG case that met the group D criteria shows some representative endoscopic findings presented in a multicenter study of AIG in Japan.
