要旨●大腸癌の前癌病変である大腸腺腫を早期に発見し切除することは大腸癌予防の観点から非常に重要である.大腸内視鏡検査は腺腫の発見において有用な検査法とされているが,1回の大腸内視鏡検査では一定の割合で腺腫が見逃されていると言われている.そこで,従来型の大腸内視鏡検査では,解剖学的に観察が難しい部位に存在する病変の見逃し率を改善させるために広視野角内視鏡(Fuse®)が開発された.Fuse®は,スコープ先端部前方および左右両側面に合計3つのCCDレンズが搭載されていることが特徴である.これによって従来の前方視野角内視鏡の2倍の視野角330°での内視鏡観察が可能となったことから,ADRの向上が期待される.
It is widely accepted that colonoscopy is one of the most effective tests for early detection and polypectomy of adenomas as it is associated with a reduction in the incidence of and mortality associated with colorectal cancers. However, reportedly, a significant percentage of adenomas can be missed during colonoscopy primarily because of poor visualization of the anatomical sites, which tend to be hidden during the standard forward-viewing colonoscopy. FUSE®(full- spectrum endoscopy)has been developed to reduce the adenoma miss rate during standard forward-viewing colonoscopy. Fuse involves three charge-coupled device lenses positioned at the front and on the sides of the colonoscope. This approach increases the maximum field of view by nearly two-fold from 140°-170° with standard forward- viewing colonoscopy to 330° with Fuse, thereby improving overall adenoma detection rates.
