要旨●大腸腺腫に対する内視鏡的摘除は,大腸癌の発生率およびその死亡率低下につながることから,広く行われている.一方で,腺腫発見率(ADR)は,検者の能力や内視鏡機器に差があることが報告されており,内視鏡検査の質の指標とされている.その向上のために,大腸の解剖学的理由から広い視野角を得ることを目的とした内視鏡の開発が進んでいる.従来の前方視レンズに加え,側後方の視野が得られるパノラマレンズを内視鏡先端に搭載した広角内視鏡(extra-wide-angle-view colonoscope)がOlympus社にて開発されている.2つのレンズから得られた画像がひとつのモニターに表示されることが特徴であり,大腸のひだ裏などのいわゆる“死角”の観察が容易となることからADR向上の期待がかかる.
Polypectomy has been accepted for adenomatous lesions in the colorectum, being associated with a reduction in the incidence and mortality of colorectal cancer. However, ADR(adenoma detection rate)depends on endoscopic skill and the quality of endoscopic images. ADR is a good quality measurement of colonoscopy. For improving its quality measurement, several novel colonoscopes have recently been developed with a view to achieve a wider angle field. Olympus has been developing the Extra-wide-angle-view colonoscope, which has two lenses:a lateral-backward view lens and a standard-angle, forward-view lens. Two views from both lenses are generated simultaneously and displayed on video monitors as a single image. This novel colonoscope has the potential to reduce the number of missed lesions where located behind haustral folds, flexures, or rectal valves, resulting in improving ADR.
