特異的な大腸内視鏡像を呈した糞線虫症の1例

Strongyloides Colitis with a Rare Colonoscopic Findings, Report of a Case

要旨●特異的な大腸内視鏡像を呈し，診断に難渋した糞線虫症の1例を経験した．患者は70歳代，男性．主訴は慢性下痢，体重減少．2年前に感染性腸炎の疑いで入院加療を受けたが，保存的加療で症状は改善し，経過観察となっていた．精査のため，大腸内視鏡検査を施行したところ，S状結腸から直腸に発赤，びらんを伴う小隆起が多発していた．診断がつかなかったため，上部消化管内視鏡検査を施行した．十二指腸粘膜の浮腫と白色絨毛を認めた．生検上，十二指腸の陰窩内および大腸の粘膜固有層に虫体を，両部位の粘膜下層に好酸球中心の炎症細胞浸潤を認め，糞線虫症と診断した．糞線虫感染による大腸炎の報告は少なく，さらに病変が左側結腸優位の症例はまれであるため報告する．

　We report a rare case of Strongyloides colitis with unusual endoscopic findings, mainly involving the rectum and left-sided colon.

　A man in his 70s was admitted to our hospital due to chronic watery diarrhea and weight loss. Colonoscopy revealed colitis with multiple erosions and small ulcers with redness in the rectum and sigmoid colon. Furthermore, upper gastrointestinal endoscopy revealed diffuse white mucosa in the duodenum and reddish mucosa in the gastric antrum. Pathological examination revealed filariform larvae of Strongyloides and eosinophilic infiltration of the lamina propria in the duodenum and colon.
