画像診断プレゼンテーションの基本手順：小腸・大腸─炎症性疾患

Expertise for Case Presentation of Inflammatory Diseases of the Small Bowel

要旨●小腸・大腸における炎症性疾患の画像プレゼンテーションについて，実例を提示しながら私見を述べた．腸管の炎症性疾患は外科的・内視鏡的切除に至ることは比較的少ない．したがって，臨床情報や画像所見の推移を参考としながら，生検病理所見を加味して診断を組み立てる作業が重要である．なかでも，肉眼所見の代替として充盈法，圧迫法および二重造影像を駆使した消化管X線造影検査で病変の分布や偏在性，腸間膜との関係，随伴病変を明示することが重要と思われる．一方，内視鏡検査では色素内視鏡検査の併用によるX線造影所見との対比と，微細な局所所見を要領よく提示すべきと考える．

　We explained in detail how to handle and present images obtained using radiography and endoscopy for case discussion of inflammatory diseases of the small bowel. Since histopathological findings of the resected specimens are not unequivocally involved in the discussion, presentation of adequate and appropriate clinical data and images is crucial for a fruitful discussion.
