小腸の非腫瘍性疾患—腸管Behçet病/単純性潰瘍

Intestinal Lesions in Patients with Behçet Disease, Simple Ulcer, and Trisomy 8

要旨●腸管Behçet病ないし単純性潰瘍と臨床診断した18例の臨床像と腸病変を遡及的に検討した．2例でトリソミー8が陽性であり，いずれも単純性潰瘍と診断されていた．全例で回盲部に打ち抜き潰瘍を認め，回腸病変を伴っていた．トリソミー8陽性例では回盲部に打ち抜き様潰瘍が多発していたのに対し，ほかの16例中8例では終末回腸に下掘れ潰瘍が認められた．トリソミー8陽性2例とトリソミー8陰性16例中3例で空腸潰瘍が認められた．以上よりトリソミー8陽性小腸病変と腸管Behçet病・単純性潰瘍の分布に差異がある可能性が示唆された．

　This study retrospectively investigated clinical characteristics and intestinal lesions in 18 patients with intestinal Behçet disease and simple ulcer. We observed two cases of trisomy 8. All 18 patients had an ulcer in the ileocecal area and ileal lesions. While patients with trisomy 8 had multiple punched-out ulcers in the ileocecal lesion, eight cases in another 16 patients had a deep ulcer in the terminal ileum. Furthermore, five patients had jejunal lesions, including two with trisomy 8 and 3 with intestinal Behçet disease. This study suggests that trisomy 8-associated enteritis might be distinct from intestinal Behçet disease and simple ulcer.
