 The diagnosis of some intestinal inflammatory diseases with the specific or characteristic histological findings can be made by the biopsy, however, it is often difficult. The causes of the difficulty are follows;1)only a small a portion, the mucosa and superficial submucosa, can be observed in the biopsy specimen, 2)when the specific or characteristic histological features are missed, 3)the similar findings can be seen in different diseases, 4)the histological changes are too mild to suggest some particular disease.

 Not only pathologists, but also clinicians understand the usefulness and limitation of biopsy diagnosis in order to use it effectively. Then, clinicians keep the differential diagnosis in mind, and should take a biopsy specimen from the appropriate site where the specific or characteristic histological findings can be expected.

 The diagnosis of the Inflammatory bowel disease is basically processed by combination of clinical and histological features, such as macroscopic, endoscopic or X-ray image, on consideration of the general condition and medical history of the patient. When the differential diagnosis is difficult, a follow-up is needed in order to see the change of disease due to the difference in the time phase.
