The preoperative identification of the horizontal extent of an early gastric cancer is important. ME (magnifying endoscopy) has been applied to the diagnosis of early gastric cancer. Recently, in addition to FICE (flexible spectral imaging color enhancement), which has been used, it came to be able to use BLI (blue laser imaging). We retrospectively analyzed 212 early gastric cancers from patients who underwent ESD (endoscopic submucosal dissection) after detailed observation by magnifying endoscopy with FICE and BLI. The numbers were 81 and 131, respectively. There was no significant difference between the two groups in horizontal margin free resection, but BLI is considered more useful for observation both microvascular architecture and microsurface structure than FICE.
