Postoperative Course of Ulcerative Colitis

潰瘍性大腸炎（UC）の術後の排便機能は概ね良好であり，QOLは術前と比べて同等以上であるとする報告が多い．その一方で，UCの術後には，回腸囊炎をはじめとした特徴的な晩期合併症を生じることがある．また，現在では選択される頻度は低いが，結腸全摘・回腸直腸吻合術を行った症例では，残存直腸の炎症再燃や癌化が問題となる．これらのUCの特徴を十分理解したうえで診療に当たる必要がある．厚生労働省の「難治性炎症性腸管障害に関する調査研究」班からは，回腸囊炎診断基準，回腸囊炎治療指針などが報告されており，UCの術後症例を診療する際の参考としたい．

　The most common long-term complication of restorative proctocolectomy with IPAA〔ileal pouch-anal（canal）anastomosis〕is pouchitis. Whereas acute pouchitis can be treated rapidly and successfully in the majority of patients, refractory and chronic pouchitis is a therapeutic challenge for patients and physicians. Total colectomy with IRA（ileorectal anastomosis）is another surgical technique for UC（ulcerative colitis）, which has been less performed since the IPAA technique has been established. In patients with IRA, relapse of inflammation and/or malignant transformation in the remnant mucosae is a serious long-term complication.
