Hybrid ESD for Colorectal Tumors

要旨　ハイブリッドESD（EMR併用ESD）は，病変周囲を切開後に必要かつ十分なだけ粘膜下層を剝離したうえでスネアリングするESDとEMRの利点を兼ね備えた手技であり，術時間の短縮，粘膜下層剝離操作に伴う穿孔予防（特に病変が正面に対峙している場合など），EMRでは一括摘除が困難な病変を確実に一括摘除できることなどが利点である．ハイブリッドESDの適応病変はESDの適応病変に準じるが，大きさからみた適応は，一括摘除が必要な最大径30mm大強までの病変である．ハイブリッドESDを行う際に最も重要なポイントは，粘膜下層剝離からスネアリングに移行するタイミングである．その目安としては，病変が切除潰瘍の辺縁から全周性に径10mm以上距離があり，粘膜下層が筋層直上まで十分に剝離され，スネア内に病変が十分に入る径20mm以下に縮んだ状態である．使用するスネアは，先端が跳ねず，筋層への押し付けが可能な硬性スネアが有用である．

　ESD（endoscopic submucosal dissection）for colorectal tumors is technically more difficult and requires considerable experience compared with EMR（endoscopic mucosal resection）. Also, procedure time for ESD is considerably longer than for EMR. Hybrid ESD is defined as the use of a snare during the final stage of the ESD procedure. The indication for ESD is present in the tumors up to 40mm in size, which are difficult to resect en bloc using a snare. In clinical practice, the choice of whether to use a snare as well as the timing of the snaring depend on the situation during the procedure and the condition of the lesion. The safety and efficacy of hybrid ESD was verified, because there were no statistical differences in the incidence of local recurrence and perforation between the hybrid ESD group and other treatment groups.
