はじめに
最近,筆者らは術前に診断しえた原発性食道胃重複癌で,しかも胃はⅡa+Ⅱc型の粘膜内に限局した早期胃癌であった1例を経験したので,その概要を述べると共に,集計しえた本邦症例と自験例を加えた50例の食道胃重複癌に関する統計的観察を加えて報告する.
A pessimistic view is apt to be held regarding the surgical correction of multiple cancers stretching over to two organs, chiefly because it is diflicult to detect them concomitantly. This is especially true in the case of mutiple cancers in the esophagus and stomach owing to their rare occurrence. In addition, since these organs are contiguous, two cancers on the respective organs are seldom found simultaneously before the radical operation, partly because attention is not paid equally to both organs, the more so when esophageal cancer is suspicious. Consequently, radical simultaneous removal of two cancers in both organs has been done only in a few cases in this country.
Recently we have come across primary multiple carcinomas in these organs in a 75-year-old man. The cancer in the stomach was an early one, type Ⅱa+Ⅱc, with m degree of depth invasion, confirmed as such before the surgical exploration. The patient is in good health one year after the radical surgery.
This experience has also induced us to investigate statistically all multiple cancers of the esophagus and stomach that have been reported in Japan. So far, 49 such cases have been collected from the literature. Together with the present one, we have studied 50 cases in detail including their predilection age bracket, difference in sex, chief complaints, duration of distress, preoperative diagnoses, locations, modes of surgical operation, histopathological findings and prognosis.
