大腸内視鏡診断学の歩みと展望

Overview of Articles on Colonoscopy Appeared in the Journal of I to Cho (Stomach and lntestine) and Gastroenterological Endoscopy

要旨　最近20年間のcolonoscopyの変遷を概観した．「胃と腸」の創刊は奇しくもcolonoscopeの誕生と時期をほぼ同じくし，その普及発展と共に歩んできたと言える．20年間の主題を振り返ってみると，大腸の主要疾患のすべてが主題に取り上げられていると共に，ポリペクトミー，健診，スクリーニング，拡大観察にまでも及んでいる．「胃と腸」は診断中心の雑誌であり，器械，検査法に関してはたとえ載っても，確立されてからのものが多い．したがってこの時期の大腸内視鏡に関するscopeの開発，挿入法は主として学会誌である「Gastroenterological Endoscopy」の論文，学会のシンポジウム，パネルディスカッションをみることによって補った．最初の10年間は内視鏡の挿入，器種の改良にエネルギーが割かれ，あとの10年ではそのscopeを実際の臨床に使って多くの成果を挙げてきたというのが大雑把な歴史である．更に近年では適応の拡大（小児スクリーニングなど）と研究面の展開（大腸生理面，動物実験など）がなされている．その間にあって，器械のたゆまざる改良・努力も見逃せない．

　In the past twenty years, the progress of colonoscopy and colonoscopic diagnosis is tremendous. The first decade was focused on the technique of colonoscopic insertion. Today every kind of colonic diseases is successfully and effectively diagnosed by colonoscopy. It has become important, not neglectable method in the diagnosis of colonic diseases. In the recent past the field of colonoscopy has extended to various fields; pediatric colonoscopy, chromocolonoscopy, mass screening examination, functional study, use for experimental animal and so on. The future of colonoscopy is excellent.
