消化管の悪性病変と皮膚

Skin Manifestation of Gastrointestinal Malignancy

　内臓の悪性腫瘍が種々の皮膚病変と密接な関係にあることは，古くから経験的によく知られている．そのような皮膚病変の中には，悪性腫瘍の皮膚転移のように，非常に特異的なものがある．しかし臨床上重要なものは，むしろ内臓悪性腫瘍の皮膚表現(skin manifestation)といった，非特異的な皮膚病変であろう．つまり，ある皮膚病変を見たときに，内臓悪性腫瘍の存在を推定させるようなものが興味があり，syndroma dermato-tumoraleという言語で一括される．または内臓悪性腫瘍のDermadromとも言うことができる．

　このような例は枚挙にいとまがなく，また，内臓悪性腫瘍の性質についても，必ずしも消化管の悪性病変だけを示唆するものではない．これをTable 1にまとめ，そのうち消化管と関係の深いものにつき簡単に記す．

　Relationship between malignant tumor of the internal organ and the skin can be classified as 1) skin metastasis of the malignant tumor, 2) non-specific skin manifestation of the malignant tumor and 3) hereditary syndrome which tends to have malignant tumor. We, however, discuss here mainly about the non-specific manifestation of the malignant tumor by showing representative cases of the dermadrome. Such cases are acanthosis nigricans, Leser-Trélat's sign, dermatomyositis, atypical erythemas (urticarial erythema, exsudative erythema, anular erythema), prurigo, pruritus and etc. Among the hereditary syndrome, on the other hand, the disease which is accompanied with polyposis―especially Peutz-Jeghers syndrome―is important.
