症例10 17歳（♂）の進行胃癌

A Case of Advanced Gastric Cancer in a 17-Year-Old Male

　昭41年4月以降，昭46年12月まで過去5年9カ月間に若年者胃癌12例を経験した．0歳～20歳に3例，21歳～30歳に9例，20歳未満は若年者胃癌全例の25％に相当する．20歳未満の3例は♂1，♀2で，♂♀各1例（Borr. Ⅱ―前庭部，早期癌Ⅱc―胃角部）が切除可能であり，他の♀1例（食道・噴門癌）は胃瘻造設術に終った．20歳未満で切除可能であった♂の進行癌の1例を記載する．生後17歳3力刀の学生で，昭42年1月26日上腹部痛とるいそうにて来院．胃X線検査で立位充盈像（Fig. 1）にて前庭部幽門近くに陰影欠損像あり，圧迫像（Fig. 2）にて前庭部後壁より小彎側におよぶ示指頭大のニッシェと，それを囲む透亮帯を証明．ニッシェの陰影は濃淡不規則．Borr. Ⅱと診断す．内視鏡像（Fig. 3）では前庭部小彎中心の底部に壊死を伴う，周囲不規則な深い潰瘍を証明，その周囲はたかまりを作りBorr. Ⅱであった．胃洗滌細胞診（GFC）にてClass Ⅳであった．切除胃肉眼的所見（Fig. 4）で幽門に近く前庭部で小彎中心前・後壁におよぶ大きなBorr. Ⅱを証明した．病理組織学的所見（Fig. 5）でAdenocarcinoma tubulareであった．若年者胃癌はその特徴として進行が成人に比し速く，発見された時は既に進行癌でしかも手術不可能例が多く．本院にても12例中切除可能は6例であった．しかも，手術成功例が陥凹型（Borr. Ⅱ-5，早期癌Ⅱc-1）であったことは興味深い．

　During the 5 years and 9 months from April 1966 to December 1971 we have come across 12 cases of gastric cancer under the age of 30 at the Tenri Hospital. Gastrectomy was performed in 6. Of 3 cases under the age of 20,2 were resectable, the one being a 18-year-old girl with early cancer of the angulus and the other, a 17 years old boy with advanced cancer of the antrum.

　The latter case is described here. He was first seen with epigastralgia and weight loss. X-ray examination revealed a filling defect in the antrum in barium-filled pictures and on compression it was seen as an irregular crater. Gastroscopy showed a protuberance with a deep, irregular ulceration compatible with Borrmann Ⅱ type. Direct-vision washing cytology yielded PAP Ⅳ cells. In the resected specimen a large Borrmann Ⅱ type carcinoma was apparent on the lesser curvature of the antrum. Histological diagnosis was adenocarcinoma tubulare.
