症例9 16歳（♀）の進行胃癌

A Case of Advanced Gastric Cancer in a 16-Year-Old Female

　患者は16歳女子，主訴は心窩部痛，既往歴は生後6カ月にヒ素ミルク中毒があるが，家族歴に特記すべきことなし．昭和46年8月頃心窩部痛，嘔吐を訴え，渡辺内科にて胃癌と診断され当院に紹介された．

　腹部所見は心窩部に圧痛ある腫瘤様の抵抗を触れ，臨床検査は血沈の亢進以外，著変なし．

　A 16-year-old girl was admitted with a chief complaint of pain in the pit of the stomach. X-ray showed a large shadow defect on the greater curvature of the lower corpus. An irregular niche was disclosed by compression in the center of this part. Endoscopy revealed a large ulcer, with convergence of the mucosal folds and narrowing of the lower corpus. Operation was done under a diagnosis of advanced gastric cancer (Borrmann Ⅲ). Its results: H₀ P₃ N₃ S₃, Stage Ⅳ as determined by the general rules of Japanese Research Society for Gastric Cancer.

　Resected stomach revealed a large, deep ulcer and irregular, hyperplastic mucosal folds in the greater curvature side of the lower body. Histologically it was predominantly carcinoma solidum simplex.
