 It has been attempted to find how accurately the depth of cancer infiltration can be estimated by gross morphology. Most reliable criteria for its estimation were also looked for, by dividing histologically gastric cancer into two categories: gastric cancer associated with peptic ulcer (ulcerous cancer) and gastric cancer unaccompanied with it (non-ulcerous cancer, mostly of protruding types).

 The subjects of this study were 400 cases of early gastric cancer and nearly 1,000 cases of advanced cancer fit for histological investigation. They were all operated on in the National Cancer Center for the past ten years.

 Ⅰ. Gross diagnostic criteria for estimating the depth of cancer invasion in ulcerous cancer are as follows:―

 1. Changes in the mucosal folds

  a. Fusion of the folds presenting a worm-eaten picture

  b. Protruded fusion

  c. Ring-like fusion

 2. Changes in the mucosal surface of Ⅱc

  a. Abnormal protrusion in or around Ⅱc

  b. Plateau-like rigidity and elevation

 Ⅱ. As for estimation of depth invasion in protruding varieties of gastric cancer, it is possible to distinguish to a certain degree between those cancers with m or sm and those penetrating into the deeper layers, provided, that not only its outer form and the nature of its surface but also its size itself be duly taken into account.
