大腸脂肪腫は比較的まれな疾患と考えられてきたが,近年その報告例は増加している.今回われわれは腸重積症を伴った大腸脂肪腫を経験したので報告する.
A 47-year-old woman visited our clinic, because of lower abdominal pain of six months' duration. Physical examination was not remarkable and no organs or mass were palpable. Laboratory studies showed a blood sedimentation rate of 64 mm in lhr. RBC was 344×104, Hb 10.0g/dl, WBC 6,200 and differential count was almost normal. Total protein was 7.0g/dl, albumin 51.1% and α2-globulin 16.5%. The chest x-ray film and EKU were normal. Barium enema showed obstruction due to intussusception with an elliptic, smooth defect in the mid transverse colon. Double contrast examination revealed reduction of the intussusception and an ovoid, pedunculated mass in the area of the hepatic flexure. An exploratory laparotomy was carried out and a pedunculated mass was found in the hepatic flexure. Histologic diagnosis was an almost ovoid submucosal lipoma, measuring about 55×60mm.
