最近10年間（1970～1979）の本邦報告例の集計からみた空・回腸腫瘍―II．良性腫瘍

Primary Benign Neoplasms of the Jejunum and Ileum: Review of the Japanese Literature

　臨床的に取り扱われる小腸良性腫瘍は，悪性腫瘍よりも一層まれな病変と考えられ，“良性”であるが故に臨床家の関心を引くことも少ない．したがってまとまった報告も多くはないが，小腸良性腫瘍に関する知識と関心が普及すれば，これらの腫瘍が重篤な合併症を来す以前に発見されることも期待される．

　今回は前回に引き続き小腸腫瘍のうちの良性腫瘍に限って本邦報告例を集計し，主として欧米の文献を参照しつつ考察を試みた．

　Primary neoplasms of the jejunum and ileum during a ten year period from 1970 to 1979 were reviewed. Among benign neoplasms, gastrointestinal polyposis such as Peutz-jeghers syndrome, familial adenomatosis of the colon, juvenile polyposis,Cronkhite-Canada syndrome were not included while small bowel diseases such as that of von Recklinghausen were And eosinophilic granuloma, cyst, aberrant pancreatic tissue, granuloma, hamartoma, polyps were listed under benign tumor-like lesions.

　1) The number of cases were as follows: benign neoplasms 214, benign tumor-like lesions 59, atotal of 273 cases. Among the benign neoplasms, there were 90 leiomyoma case, 37 lipoma, 24 angiuma, 21adenoma, 6 neurogenic tumors, 3 lymphangioma. For benign tumor-like lesions, there were 9 cases of eosinophilic granuloma, 4 hamartomas, cyst 4, aberrant pancreatic tissue 16, polyps 18, Others 8.

　2) 678 cases of malignant tumors were reported in Japan during the same period with a 1: 3.2 ratio for benign to malignant cases indicating a majority of malignant. This is partly due to the fact that most of the cases found by coincidence at autopsy were not included.

　3) Sex ratios were 1.2: 1 for benign tumors and 1.7: 1 for malignant showing higher frequency in males.

　4) Age distribution: Higher frequency were seen in those of the sixty years old age group and 40% of the whole were fifty and sixty years old age groups.

　5) Tumors size: 70% were less than 5cm.

　6) Location: 57.7% were found in the jejunum and 43.3% in the ileum. Mureuver, leiomyoma, adenoma, neurogenic tumor, fibroma were found in the jejunum and most of the lipomas in the ileum.
