小腸腫瘍―最近5年間（1995～1999）の本邦報告例の集計

Primary Small Intestinal Tumors

要旨　1995～1999年，5年間の小腸原発性腫瘍の報告例を医学中央雑誌より抽出，集計し，20年前の集計と比較した．(1)原発性悪性腫瘍の組織型別頻度は癌157例，悪性リンパ腫146例，平滑筋肉腫140例，悪性神経原性腫瘍8例，カルチノイド8例，その他22例であった．(2)20年前の集計に比し，各組織別悪性腫瘍の明らかな増加傾向はなかった．(3)病変の存在部位は20年前の集計よりも良・悪性ともTreitz靱帯およびBauhin弁より離れた部位の病変が多い傾向があった．(4)GISTにおける免疫染色が行われた報告は1998年度以降では約半数であった．(5)そのほか腫瘍の大きさ，臨床症状など20年前の集計と大差なかった．

　We have reviewed Japanese case reports of primary small intestinal tumors which have been cited in the Igaku Chuoh Zasshi in the recent five years (1995～1999).

　These reports (the new series) were analyzed and compared with case report summaries 20 years ago (the old series). The results were summarized as follows; 　1) Primary malignant tumors classified according to histology and relative frequency in the new series were; cancer 157, malignant lymphoma 146, leiomyosarcoma 140, malignant neurogenic tumor 8, carcinoid 8, Kaposi's sarcoma 1 and others 21.

　2) A comparison with the series 20 years ago revealed that relative frequency of each malignant tumor in the new series was not different from that in the old series.

　3) A comparison of tumor location with that of the old series revealed that tumors located apart from the Treitz ligament and tumors located apart from Bauhin's valve in the new series were more numerous than in the old series.

　4) Concerning histochemical staining for GIST, cases which used this staining comprised more than half of the cases after 1998.

　5) A comparison of other factors revealed that tumor size and clinical symptoms in the new series were not different from those of the old series.
