  • Look Inside

To investigate the histological changes of the early lesions of Crohn's disease, the operation materials of a 30 year-old female patient who underwent operations three times in six years because of recurrence were studied using a scanning electron microscope.

 The following results were obtained.

 1) The longitudinal fissuring which is 10 to 20 μm in width is seen at the bottom of the ulcer.

 2) The villi surrounding a big ulcer are atrophic, the surface like a pumice with furrows which have vanished, and the orifices of goblet cells are widely open.

 3) As for the villi around a small ulcer, the furrows are deep and orifices of goblet cells are deep and large.

 4) The villi of the oral-side intestine which was biopsied during operation are atrophic, and their histological changes resemble the lesion even though the changes are lighter.

As Crohn's disease aggregates, the surface of villi changes as described. The mucosa which seems to be macroscopically normal has similar light changes like the lesion when they are investigated using a scanning electron microscopy and this may suggest the cause of high recurrence rate of Crohn's disease.
