  • Look Inside

A 47-year-old woman was admitted to Jichi Medical School Hospital on August 2, 1974, with tarry stools. Radiographic and endoscopic examinations failed to detect the bleeding site in the gastrointestinal tract and she was discharged. The patient was readmitted to the hospital in preshock with tarry stools on July 12, 1977. She recovered from the shock after receiving 2,000 ml blood transfusion. Selective superior mesenteric arteriogram revealed a spot of contrast medium in the jejunum, which was thought to be compatible with arteriovenous malformation of the jejunum. On emergency operation, a pulsating tumor in 6×3 mm size was found in the jejunal wall about 80 cm distal to the ligament of Treitz. Intraoperative endoscopic examination showed a submucosal tumor with coagula at its apex. The tumor was removed and examined histologically. The tumor consisted of irregularly dilated thick-walled vessel and small arteries. Arteriovenous malformation of the jejunal wall was confirmed.
