 A 60-year-old housewife was examined for gastric cancer without any symptoms.

 By roentgenography and endoscopy, a protruding mass on the lesser curvature near the esophagogastric junction was revealed and diagnosed as cancer (Fig. 1). Total gastrectomy with distal pancreatectomy and splenectomy was performed on Jan. 7, 1987. Resected specimen showed macroscopically a protruding, well delimited mass with central ulceration (4.7 × 2.8 cm in size) on the mucosa (Fig. 2) and yellowish solid lesion involving mucosa and proper muscular layer on the cut surface (Fig. 3). However, histological examination disclosed that the lesion consisted, for the most part, of histiocytes with foamy cytoplasm and fibroblasts (Fig. 4), and only a few small foci of carcinomas with lymphocyte infiltration were scattered in the xanthogranuloma tissue (Figs. 6 and 7).

 This is believed to be the first recorded instance of xanthogranuloma accompanying carcinoma in the same tumor of the stomach.
