早期大腸癌の肉眼的分類―早期胃癌肉眼型分類との整合性を求めて

Macroscopic Classification of Early Colorectal Cancer, Seeking for Consistency with that of Early Gastric Cancer

要旨　人体における主要な種々の実質臓器や管腔臓器の癌取扱い規約において，癌の肉眼分類を比較検討した．この検討を踏まえ，早期大腸癌の肉眼分類について，①分類学と形態学，②管腔臓器であることに由来する腫瘍の形，③予後や悪性度との相関，④表在型癌における治療法の選択，⑤食道癌，胃癌との整合性，などの視点から検討した．その結果，早期大腸癌の肉眼分類は，純粋に側面からみた凹凸だけで分類されていることの重要性を再確認した．そして現在，特に問題になっている表面陥凹型，およびその混合型の早期癌の肉眼分類についても，表面隆起の部分における腫瘍の有無で決定すべきではない．すなわち肉眼分類である以上，病理学的な要素を考慮しないで，側面の形を考慮した肉眼の形態のみで分類すべきである．

　For the purpose of considering macroscopic classification of early colorectal cancer, macroscopic classifications of various solid and luminal cancers were compared from the following viewpoints : 1) taxonomy and morphology, 2) a shape of the tumor which was associated with the luminal organ of which the cancer arises from the internal surface, 3) correlation of prognosis and degree of malignancy, 4) treatment option of the superficial cancer, 5) consistency with esophageal and gastric cancers. In conclusion, I re-confirmed the important point that macroscopic classification was purely based on the unevenness of the lateral view of the lesion. The current controversy in the macroscopic classification of superficial depressed type and mixed type early colorectal cancer should not be fixed by the existence of the tumor on the surface of the elevated area. Namely, macroscopic classification should be based on the shape of lateral view of the lesion irrespective of the pathologic component.
