食道粘膜癌と粘膜下層癌―病態の相違からみて

Histopathological and Molecular Biological Characteristics between Mucosal Cancer and Submucosal Cancer of the Esophagus

要旨　食道癌は粘膜筋板を境にして粘膜癌(M癌)と粘膜下層癌(SM癌)に分類されるが，これら両者には非常に大きな生物学的相違がある．SM癌になるとリンパ節転移が一挙に40％と高率になり，予後もM癌に比較し有意に不良となる．この原因について，分子生物学的な観点から解析を試みた．食道の粘膜下層では脈管の発達が著しくなり，SM癌ではリンパ管や血管への侵襲が増加する．また，SM癌では細胞間接着分子(E型カドヘリン)の発現が減弱し癌細胞が容易に解離するようになり，cyclin D-1の遺伝子増幅例では血行性転移を高頻度に生じる．粘膜下層という環境因子が癌細胞の機能に何らかの変化を招来している可能性が示唆された．

　The incidence of positive lymphatic invasion or lymph node metastasis tended to increase markedly as cancer infiltrated the lamina muscularis mucosa. In molecular biological behavior, intercellular adhesion molecule, E-cadherin, showed reduced expression in submucosal cancer by immunohistochemical staining, while cyclin D-1 gene amplification was more frequently seen in submucosal cancer than mucosal cancer.

　These molecular biological and histopathological characteristics of esophageal submucosal cancer may explain its significantly worse prognosis as compared to mucosal cancer.
