serrated adenomaからの癌化と思われる大腸進行癌の1例

An Advanced Cancer Derived from a Serrated Adenoma of the Colon, Report of a Case

要旨　患者は64歳，女性．便潜血陽性のため施行した注腸・内視鏡検査で，横行結腸に台形状の壁変形を示し，口側に7mmの発赤調のポリープを伴った10mmの粘膜下腫瘍様の隆起性病変を認め，横行結腸切除術を行った．病理組織学的検索によると病変はserrated adenomaから発生した癌で，その深部浸潤により粘膜下腫瘍様隆起が形成されたと考えられた．癌の浸潤はssにまで及んでいた．serrated adenomaの癌化によると思われる進行癌の症例の報告は少なく，若干の文献的考察を加えて報告する．

　The case was that of a 64-year-old male who visited our hospital further examination of positive occult fecal blood. An elevated area, which resembled a submucosal tumor, with a reddish colored polyp was detected at the transverse colon where the deformity of the colonic wall was also noticed. The lesion was resected by a partial colectomy. The lesion was 17 mm in size and was a well-to-moderately differentiated adenocarcinoma which had invaded beneath the subserosa.
