 We often have difficulty in diagnosis of gastric biopsy because of the smallness and compression of the sample. In case of differential diagnosis of malignant lymphoma, it is important to detect the reactive germinal centers and clarify the histologic architecture. After detecting the reactive germinal centers which are immunohistochemically CD 10 (+), bcl-2 (-) and Ki-67 (+), the atypia of infiltrating lymphoid cells around the germinal centers should be evaluated on H・E section.

 When the infiltrating cells are relatively large, carcinoma should be ruled out by immunohistochemical stain using cytokeratins. When they are relatively small, mantle cell lymphoma should be ruled out by immunohistochemical stain using cyclin D1. In addition, Ki-67 is also useful for evaluating the grade of lymphoma.

 IgH rearrangement by the PCR method is not always positive for lymphoma and it can also be positive for chronic gastritis. p53 mutation has low incidence in lymphoma. These molecular methods may be helpful for diagnosis of lymphoma, however, they should be carefully evaluated.
