 The differences of H. pylori positive peptic ulcer between young subjects(less than 30 years)and middle-aged or older ones were investigated using the data of a large-scale clinical study in Yamagata prefecture and endoscopic and histological data in the Hokkaido University Hospital. The proportion of H. pylori positive ulcer is higher and that of NSAIDs ulcer is lower in young subjects than in the middle-aged or elderly subjects. Young subjects have mainly antrum predominant gastritis and duodenal ulcers. On the other hand, corpus-predominant gastritis increases and also corpus ulcers in the middle-aged and eldely are increased. Higher rate of ulcer recurrence after H. pylori eradication in the elderly is associated with deep ulcer, NSAIDs use, and a background of diabetes. Therefore, careful treatment and endoscopic surveillance is necessary in these patients even after successful of eradication.
