4）食道表在型腺様囊胞癌の1例

Superficial Adenoid Cystic Carcinoma of the Esophagus, Report of a Case

要旨　患者は67歳，女性．味覚異常を自覚したため当センターを受診した．上部消化管内視鏡検査を施行したところ，胸部下部食道に粘膜下腫瘍様の隆起性病変を認めた．SM深部浸潤した食道腺様囊胞癌と診断し，外科的に食道切除術を施行した．病理組織学的診断は，食道腺様囊胞癌，深達度SM3，脈管侵襲（－），リンパ節転移を認めなかった．免疫組織化学的染色でも唾液腺腺様囊胞癌における充実・管状型と同様の染色態度を示し，腺様囊胞癌として矛盾しない症例であった．

　A 67-year-old woman consulted our center because of an abnormal gustatory sensation. Upper gastrointestinal endoscopy was carried out and a polypoid lesion resembling a SMT（submucosal tumor）was recognized in the lower thoracic esophagus. We diagnosed it as adenoid cystic carcinoma of the esophagus massively invading the submucosa. Esophagectomy was carried out. The operated specimen showed adenoid cystic carcinoma of the esophagus, SM3, ly0, v0, but LN metastasis wasn't recognized. Even immunohistochemical dyeing showed the same dyeing characteristics as a solid and tubular type adenoid cystic carcinoma in the salivary glands, and it didn't contradict current descriptions of adenoid cystic carcinoma.
