 We reviewed AFP-producing gastric carcinoma and its related entities in terms of their expression of novel oncofetal proteins, GPC3(glypican3)and SALL4. AFP, GPC3, and SALL4 are simultaneously expressed in the fetal gut in early gestation and a part of primitive germ cell tumors, suggesting that these proteins are fetal differentiation markers in the gastrointestinal tract. AFP-producing gastric carcinomas invariably expressed these markers, and they are positioned as gastric carcinoma of fetal phenotype. Fetal gut-like and hepatoid adenocarcinomas expressed GPC3 and/or SALL4 regardless of AFP-production. “Fetal adenocarcinoma", a distinct subgroup of gastric carcinoma of fetal phenotype, including AFP-producing, fetal gut-like, and hepatoid types, can be extracted by using GPC3 and SALL4, in addition to AFP, as specific markers.
