Crohn病小腸病変に対する治療効果―栄養療法の粘膜治癒効果

The Impact of Enteral Nutrition on Mucosal Inflammation in Patients with Crohn's Disease

要旨　Crohn病の治療において，免疫調整剤やインフリキシマブに粘膜の炎症を消失させる効果，いわゆる粘膜治癒効果があることが最近報告されている．経腸栄養療法は，本邦ではCrohn病患者に対する第1選択の治療法として提唱されているが，その粘膜治癒効果についてはこれまで十分な評価はなされていなかった．筆者らは，活動期および寛解期のCrohn病患者を対象に臨床試験を行い，経腸栄養療法の臨床的および内視鏡的有効性を検証した．その結果，経腸栄養療法が活動期，寛解期いずれのCrohn病患者においても，臨床的にも内視鏡的にも活動性を抑制することが証明された．

　In the management of CD（Crohn's disease）, recent studies have shown mucosal healing as the disappearance of endoscopic inflammation in patients treated with immunosuppressive drugs or infliximab. Although enteral nutrition（EN）is advocated as a primary therapy for patients with CD in Japan, the impact of EN on mucosal inflammation has not been fully assessed yet. We have conducted a number of clinical trials in order to evaluate the clinical and endoscopic efficacy of EN with an elemental diet in patients with active and quiescent CD. We found that EN clearly suppressed clinical and endoscopic disease activity in patients with both active and quiescent diseases.
