要旨　経肛門的内視鏡下マイクロサージェリー（TEM）を施行した早期直腸癌は116例であった．肛門縁から病変の距離は5～19cm，病変径は最大140mm（平均42.1mm）であった．手術時間は25～300分（平均90.8分）であった．標本径は最大150mm（平均59.0mm）であった．一括切除率は96.6％であった．術中術式変更は1例のみであった．術合併症は篤なものは経験しなかった．深達度はM～SM1 108例，SM2 4例，SM3 4例であった．5例（4.3％）に追加切除を行った．2007年までの104例において直腸癌再発死亡例は3例（2.9％）であった．

　Transanal endoscopic surgery was performed in 116 patients with early rectal cancer. The distance between the tumor and the anal verge ranged from 5 to 19cm. The diameter of the tumor was 42.1mm（average）and 140mm（max）. The operation time ranged from 25 to 300 minutes with an average of 90.8 min. The diameter of the resected specimen was 59.0mm（average）and 150mm（max）. En-bloc resection was successful in 96.6% of the cases. Change of the operation method was needed in one case. No significant operative complication was noted. The invasion depth ranged between M-SM1（108）, SM2（4）, and SM3（4）. Complementary surgery was performed in 5 cases. Follow up in 104 cases revealed death through recurrence in 3 cases.
