経過観察例からみた胃癌ハイリスクグループ―職域内視鏡検診の立場から

A Study of a High Risk Group for Gastric Cancer Through a Review of Followed-up Cases : From the Standpoint of Endoscopic Mass Screening of Employees

要旨　当院で1998～2007年度の10年間に実施された社員ドック（検診）の際の上部消化管内視鏡検査件数は97,007件であった．胃癌は138例発見され，そのうちで経過観察中に発見された症例は65例であった．背景粘膜には癌発見以前より全例に慢性萎縮性胃炎があり，しかも萎縮の程度は癌発見前，癌発見時ともにほとんどがC-3（木村・竹本分類）以上であった．したがって，内視鏡検査医としては確実に萎縮所見，なかでもその拡がりの程度を読み取ることが大切であり，その時点で悪性所見を見い出せなくとも，胃癌ハイリスクグループとして経過観察（再検査）が必要であることを銘記すべきである．

　This study examined the results of mass screening for gastric cancer among employees using endoscopy for the fiscal years 1998 to 2007. Endoscopic examinations totaled 97,007 cases. As a result, 138 tested positive for gastric cancer. 65 of these cases were involved in a follow up study. The mucosal background had an atrophic change before detection of the gastric cancers．In addition, the degree of atrophy in most cases was greater than C-3 grade. Endoscopists should read the mucosal atrophic change as carefully and exactly as possible. Even if they cannot detect the malignancy at the time of initial examination, they should always carry out a careful follow-up study.
