要旨　2002年1月から2008年5月までに東京大学医学部附属病院にて，食道扁平上皮癌に対する内視鏡的粘膜下層剥離術を施行された81症例104病変を対象に，長期予後の検討を行った．中央値665日の観察期間において，EP・LPM病変では原病死を認めず，また，MM以深病変でのcause-specificの5年生存率は84.9％であった．しかし，脈管浸潤陰性での一括完全切除を得た後に遠隔転移を認めたMM病変の症例もあり，MM病変についてはESDで根治が期待できるものの，術後の慎重な経過観察も必要であると考えられた．

　We investigated clinical outcomes of endoscopic submucosal dissection（ESD）for superficial esophageal squamous cell carcinoma（SCC）in our department. Eighty-one patients with 104 lesions were enrolled in this study and were treated from January 2002 to May 2008. With a median follow-up period of 665 days, cause-specific survival rates of T1a-EP/T1a-LPM and T1a-MM/SM at 5 years after ESD were 100% and 84.9%, respectively. However, lymph node metastases were observed in a patient with T1a-MM SCC even after curative resection without any lymphovascular invasions. These facts indicate the curative potential of ESD for superficial esophageal SCC that invades the muscularis mucosa and the necessity of careful observation after ESD.
